  2. What is most important to me?
  3. Do I have any ideas about how to get to the goal?
  4. What am I good at?
  5. What can I do?
  6. What do I need help with to reach my goal?
  7. Remember, you have services and supports that can help you. Ask what can be in place to help you.
  8. Would it help to write things down to talk about?
  9. Can you talk to your RC about what you want to talk about before the meeting?

The Service Plan meeting:

  1. Bring up what you want to talk about. Bring a list.
  2. Have the team talk about your concerns and ideas completely.
  3. If you don’t understand, ask questions or say, “I don’t understand.”
  4. Goals – these should be what YOU want
  5. ISSPs – these are the programs day and residential are to work on with you. Do they help me reach my goals?
  6. Be responsible. Tell people what you can do to help yourself.
  7. Hold people responsible. Make sure it is in writing. Who is going to do what? When can they do this by?

What do I want to talk about at my meeting? Make a list of things you want to talk about at your meeting prior to attending (ie. home, school, work, family, friends, activities, fun things)
